IN GOOD COMPANY PET TRANSPORTATION SERVICES

Pet Transportation Agreement

*      The animals being transported are no less than 8 weeks old and have a valid health certificate signed and issued by a licensed DVM.

*      The animals being transported have received rabies vaccine if 16 weeks of age or older.

*      I will provide 3-7 days supply of food for my pet for the duration of transportation.  I will include all feeding schedules and instructions as applicable to the feeding of the pet(s) or animal(s) being transported.

*      By signing this agreement, I give permission for emergency care to be provided to the animal or animals being transported if such a need arises.  I agree that all hospital costs, veterinary services, and other services related to such an emergency will be paid for in full.  I agree that I will not hold In Good Company Pet Transportation Services, or its agents responsible for any illness, injury, or loss incurred during transport of my pet(s) or animal(s).

*      I will provide at least 2 phone numbers at which I can be reached for communications regarding my pet or animal during transport.

*      If the animal(s) being transported do not have their appropriate health papers and signed transport agreement ready by time of pickup, I agree that transport may have to be rescheduled and I may have to pay additional fees to have my pet’s transportation rescheduled.

*      As the shipper, I will provide accurate destination address, accurate contact phone numbers for the person or persons receiving the animal(s) being transported.  If no one is available to receive the animal(s) being delivered, additional fees will be incurred at a rate of  $15 per day.

*      If there is no person available at time of pickup,  In Good Company Pet Transportation Services, reserves the right to continue without your pet(s) or animal(s).  As the shipper, you will not be refunded your fee and a new pickup and delivery schedule must be prepared, with transportation fees and related paperwork.  Please be sure that you or someone you trust will be available at time of pickup to prevent rescheduling and loss of transportation fees.

*      I certify that the pet(s) and animal(s) that I am having transported are not aggressive, vicious, or dangerous.  If my pet or animal being transported does become aggressive, I understand that I may incur additional fees if my pet has to be expedited to its destination as a result.  If injury occurs to In Good Company Pet Transportation Services or any of its agents as a result of said behavior, I agree that I will be responsible for any medical bills incurred.

*      If I must reschedule or cancel, I agree that I will do so within 48 hours of the estimated pickup and/or delivery of my pet(s) or animal(s).   If proper notice is given, you will not incur additional fees for rescheduling. No refunds for cancellations.

*      In Good Company Pet Transportation Services and its agents will make every effort to provide you with an accurate estimate and an accurate schedule.  I understand that these estimates are tentative and subject to change.

*      I agree to pay all costs in full for transportation services prior to pickup of pet(s) and animal(s) being transported.  I agree to pay any additional fees as outlined in this agreement, and I agree to the terms and conditions of the agreement.
